Recertification Application

When applying online, you will need to have the following information at hand:

1. Your CPAN® and/or CAPA® credential (certification) number (located on your ABPANC wallet identification card)
2. Your ASPAN membership number if you wish to receive the ASPAN member fee discount
3. Your RN license number and expiration date
4. Your credit card information

If you are selected for audit, you will need to have complete contact information for two individuals who will be contacted to verify you have met the RN Licensure and Clinical Practice Requirement, in addition to the information listed above. Contact information required is name, credentials, title, name of institution, name of unit, address of institution, contact's phone number and email.
